Bempedoic acid is a lipid-lowering medication used to reduce elevated LDL cholesterol levels in adults with hypercholesterolemia or mixed dyslipidemia. It is especially useful in patients who are unable to tolerate statins or who require additional LDL-C lowering beyond what statins provide. The drug works by inhibiting ATP-citrate lyase, an enzyme involved in cholesterol synthesis in the liver. It is often used in combination with statins or other lipid-lowering therapies.

Dosage and direction
The recommended dose of bempedoic acid is 180 mg taken once daily with or without food. Tablets should be swallowed whole and not crushed or chewed. It can be taken at any time of day but should be taken consistently. Regular lipid monitoring is recommended to assess effectiveness.

Precautions
Before starting treatment, inform your doctor if you have a history of tendon disorders, gout, or kidney disease. Bempedoic acid may increase uric acid levels and the risk of tendon rupture, particularly in older adults or those on corticosteroids. Caution is advised in patients with severe hepatic impairment. This medication is not intended for use during pregnancy or breastfeeding.

Contraindications
Bempedoic acid is contraindicated in individuals with known hypersensitivity to the drug or its components. It should not be used during pregnancy or in patients with severe liver disease. Avoid use in people with active gout or high uric acid levels unless closely monitored. Safety and efficacy have not been established in pediatric populations.

Possible side effect
Common side effects include muscle spasms, back pain, abdominal pain, and elevated liver enzymes. It may also cause an increase in uric acid levels, leading to gout flares. Rare but serious side effects include tendon rupture or hypersensitivity reactions. Contact a healthcare provider if you experience joint pain, swelling, or signs of liver problems.

Drug interaction
Bempedoic acid may interact with statins, especially simvastatin and pravastatin, increasing the risk of myopathy. Use caution when combining with other cholesterol-lowering agents or drugs that raise uric acid levels. Always inform your healthcare provider about all medications and supplements being taken. Dose adjustments may be necessary when used with certain statins.

Missed dose
If you miss a dose, take it as soon as you remember. If it’s nearly time for your next dose, skip the missed dose and continue with your regular schedule. Do not take two doses at once. Adherence to daily dosing is important for maintaining cholesterol control.

Overdose
In case of overdose, symptoms may include muscle pain, fatigue, or gastrointestinal discomfort. Seek immediate medical help. Treatment is supportive and based on symptoms. No specific antidote exists.

Storage
Store at room temperature, away from moisture, heat, and direct light. Keep the medication in its original packaging. Do not use beyond the expiration date. Keep out of reach of children and pets.
